29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, or changes to facilities: a- Hess Port Reading Refinery - Operating procedures from 2005, 2006 and 2007 were certified as current and accurate when many of them to include but not limited to 15-5.264 P-501A/B Raw Oil Charge Pump Operation, 15-5.511 Loss of the Wet Gas Compressor and 15-5.505 Loss of Main Column Circulation required changes. Condition noted on or about 03/11/09.
