29 CFR 1915.158(b)(5): There was not at least one portable or permanent ladder in the vicinity of each floating vessel on which work was being performed: a. On or about January 21, 2018, on the pier across from the fabrication shop, an employee was allowed to work on the Tug Santaquin without a ladder in the vicinity of the vessel, exposing the employee to a drowning hazard. b.
