29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: a- 5th Floor, Elevator Shaft Entrance - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to fall hazards, in excess of 6 feet to lower levels, when working/walking next to two pieces of wood that were set up diagonally in the opening of the elevator shaft. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
