29 CFR 1910.119(j)(4)(iv): The employer did not document each inspection and test that has been performed on process equipment: On or about March 17, 2015, and at times thereafter, the employer exposed employees did not document hydrostatic testing performed on process equipment. There was no documentation maintained to identify the date of the test, the name of person performing test, serial number or other identifier of the equipment tested, description of test performed, and results of test.
