29 CFR 1910.165(b)(1): The employee alarm system did not provide warning for necessary emergency action as called for in the emergency action plan, or for reaction time for safe escape of employees from the workplace or the immediate work area, or both: On or about March 6, 2013, an explosion and fire occurred at the Fox Valley Systems, Inc., facility, located in Cary, Illinois. The employee alarm system did not provide warning for necessary emergency action, during this incident, as called for in the emergency action plan as required by 1910.119(n) and 1910.38. DOCUMENTATION AND CERTIFICATION ARE REQUIRED FOR THIS ITEM.
