29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about October 8, 2014, and at times prior thereto, the employer did not provide machine guards to the following equipment, exposing the employees to the hazards of being caught in rotating parts. a. In the Metal Storage Area on the grinding wheel, the guard was loose and not covering the grinding wheel. b. In the Metal Storage Area on the Jet Belt Sander and Disc Grinder, the bottom work rest and guard were missing. c. In the Machine Shop on Standard Lodge & Shipley lathe #21, the chuck guard cover was not installed on the lathe. d. In the Machine Shop on lathe #35, the chuck guard cover was not lowered into position while the operator was running the lathe. e. In the Machine Shop on lathe #38, the chuck guard cover was missing. f. In the Heat Treat Department on the HEM Saw, the saw did not have guards installed over the non-working portion of the blade. g. In the CNC Department on the #2 Cadet CNC machine Model #L1420, interlocks had been by-passed, permitting access to the rotating chuck. h. In the CNC Department on the #6 Hwacheon CNC machine Model #Hi-ECO-45, interlocks had been by-passed, permitting access to the rotating chuck. i. In the CNC Department, on the #4 Moriseiki LL-7, the Chip Conveyor belt was missing the guard cover over the ends of the ejection area. j. In the CNC Department on the #3 Moriseiki SL-7, the Chip Conveyor belt was missing the guard cover over the ends of the ejection area.
