29 CFR 1910.215(b)(9): Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, were not constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. A.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA, 15824, production area, on or about November 23, 2015 and times prior thereto: Employees utilize the DeWalt DW756 6-inch bench grinder for maintenance type tasks including, but not limited to, grinding down bolts. The tongue guards were affixed to the grinder on both sides, however the tongue guards were installed backwards and not being utilized.
