29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process: a- Bayway Chemical Plant, Dispersants Unit - An MOC was not written to allow fire hydrant water to be used for the 8J22 Scrubber until jets could be unplugged and finer mesh screen installed in the strainer. Condition noted on or about 01/27/10 and 02/09/10. b- Bayway Chemical Plant, Piping Loop LMAM01004-09, Vis-J Unit - An MOC was not written when TML 19 (19.01-19.06) on a 4 inch elbow were changed to an adjacent elbow due to difficulty in accessing the TML. Condition noted on or about 12/22/09.
