29 CFR 1910.212(a)(2): Guards were not affixed to the machine(s) when possible a nd secured elsewhere when attachment to the machine(s) was not possible. The emp loyer does not ensure that the bolts to secure a drive shaft cover guard from th e Naphtha Pump 31502 to a Siemens Motor (#2144282002-2) are installed. The pump is located in the #2 Platformer Unit. All four bolts were missing from this guar d. Every other drive shaft cover guard observed by the CHSO had at least one mis sing bolt. This violation was observed on or about 5/10/10. Employees would be e xposed to a struck-by hazard if the guard suddenly shifted while they were in th e area. Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation that it is in compliance with the standard, including describing the steps it is taking to ensure employe es are not exposed to this struck-by equipment hazard. One method of abatement a mong others is to attach the bolts to the cover.
