29 CFR 1910.119(f)(1)(iv): The employer's written operating procedures did not address requirements for the safety systems and their functions: a- Hess Port Reading Refinery - The employer did not list in their written operating procedure specific safety systems to include but not limited to 15-5.516 FCCU Emergency Shutdown and 15-6.002 Oxygen Free Gas Con After A Turnaround. Such systems included but were not limited to pressure relief valves, safety critical instruments and fire protection systems. Condition noted on or about 03/05/09.
