29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: On or about March 18, 2025, the employer did not ensure that knockouts on the right side of 208V/250 Amp main service connect switch were closed, to prevent dust, debris, and moisture from entering into the panel. This condition exposed employees to electrical hazards.
