29 CFR 1915.158(b)(2): On floating vessels under 200 feet (61 m) in length, at least one 30-inch (0.76 m) U.S. Coast Guard approved ring life buoy with line attached was not located at the gangway: a. On or about January 21, 2018, on the pier across from the fabrication shop, employees were allowed to access the vessel, Tug Santaquin, using a gangway that didn't have a buoy attached to the gangway, exposing employees to a drowning hazard. b.
