29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) At the worksite, on or about 5/9/17: In the stairwell, on the first landing between the first and second floors, was an unguarded hole where the lower level was 6 feet, 3 inches. b) At the worksite, on or about 5/9/17: On the second floor, at least 6 unguarded holes were maintained where employees worked in close proximity.
