29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) Mill department slitter machine: A control box for the slitter had a torn strain relief, on or about 5/16/16. b) Mill department mill room 3: An extension cord powering a pedestal fan had a torn strain relief on both ends of the cord, on or about 5/16/16. c) Mill department Kobe warm up mill: One of the electrical lines that operates the safety bar had a torn strain relief, on or about 5/16/16. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
