On or about May 12, 2016, the employer failed to guard the horizontal shafts in Building 155 B-Line, Column H:10, the white control box exposing employees to caught in hazards. On or about May 12, 2016, the employer failed to guard the horizontal shafts in Building 129, the shafts for the turning rolls.
