29 CFR 1910.307(c)(2)(i): Equipment in hazardous (classified) location(s) was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present: On or about August 5th, 2024, employees engaged in commercial bakery production activities located at 4657 G Street, Omaha, Nebraska 68117 were exposed to fire and explosion hazards in that electrical equipment located in the Roll-in room was not approved for the environment employees work in - Flour dust was present and was tested as explosive in the Roll-in room. Instances of ignition source failure(s) include but are not limited to: a) Two 120V outlets did not have approved faceplates, did not have approved flip covers, and they were not sealed -located on laminator #2. b) Flexible cord going into Mixer #4 control box was frayed, not sealed, and pulled out of the connector. c) Conduit on Laminator #1 was not sealed, and separated from connector - Near the decline side of the conveyor. d) Junction box located above the Sheave Rollers was open, not sealed, and exposing energized conductors. e) Conduit on wall behind the Roll-in line Electrical panel #2 was not sealed, and was separated from energized conductors on the top and bottom connectors. f) Junction box was not sealed, open, and exposed energized conductors between the sifter and mixer #4. g) Electrical energized cords were dangling and not sealed on Mixer #3 control panel. h) Motor behind conduit was not sealed, and was exposing energized conductors. i) Energized conductors were free floating on Mixer #2. j) Electrical Enclosure mounted on Mixer #5 was not sealed, closed, and had a hole on the top part of the door. k) Junction Box was open on laminator #2, not sealed, and exposing energized conductors. l) Two junction boxes located directly below Roller #3's conveyor has missing covers, were not sealed, and energized conductors were exposed. m) Control Panel Mounted on the side of Mixer #2 was open, not sealed, and exposing energized conductors.
