29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annu al maintenance check: The fire extinguisher maintenance program implemented by t he employer does not ensure that all portable fire extinguishers receive an annual maintenance check. Employees w ho are required to use the extinguishers are exposed to fire and explosion hazar ds as a result of flames that may spread rapidly from attempting to use defectiv e equipment. This violation was observed in the Platformer Unit on or about May 06, 2010. Discharged portable fire extinguishers are located in the following ar eas: A.A discharged Ansul Model LR-30-D Fire Extinguisher at the top of the Numb er 1 Platformer Separator. No inspection tag to indicate an inspection date is l ocated on the extinguisher. B.A discharged Ansul Model LR-30-D Fire Extinguisher near the Number 2 Platformer Stabilizer Overhead Accumulator. An inspection tag indicates the extinguisher was last checked in 2004. Pursuant to 29 CFR. 1903.19, within ten (10) calendar days of the date of this c itation, the employer must submit documentation that it is in compliance with th e standard, including describing to steps it is taking to ensure employees are n ot exposed to this fire hazard. One abatement method among others is to map the locations of all fire extinguishers and maintain the equipment as required on an annual basis.
