29 CFR 1910.305(b)(2)(i): Covers of outlet boxes having holes through which flexible cord pendants passed were not provided with bushings designed for the purpose nor did the holes have smooth, well-rounded surfaces on which the cords could bear: The employer does not ensure an electrical panel and outlet were equipped with faceplate or covers. a) This violation was observed on or about May 30, 2019, in the carpenter shop, an outlet was observed missing a cover, exposing employees to electrical hazards. b) This violation was observed on or about May 30, 2019, in the overhaul shop, an outlet was observed missing a cover, exposing employees to electrical hazards. c) This violation was observed on or about May 30, 2019, in the maintenance shop, a ceiling mounted pendant box was observed missing a cover. Conduit extending from the pendant box was used by an employee to hang his uniforms from a wire hanger, exposing employees to electrical hazards. d) This violation was observed on or about May 31, 2019, in Fort Frontier by the Brew house, an electrical outlet was observed missing a cover, exposing employees to electrical hazards. e) This violation was observed on or about May 31, 2019, in the Fort Frontier by the Chuck Wagon stand, an electrical outlet was missing a cover/faceplace, exposing employees to electrical hazards. f) This violation was observed on or about June 4, 2019, in the Upholstery shop, an electrical outlet was observed missing a cover, exposing employees to electrical hazards. g) This violation was observed on or about June 4, 2019, in the Diamond Back electrical room, a small wall mounted electrical box and a small box located inches off the floor were missing covers, exposing employees to electrical hazards Pursuant to 29 CFR 1903.19, within (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure all junction boxes and each outlet box are provided with an approved covers or faceplates.
