29 CFR 1910.255(b)(6): All foot switches were not guarded to prevent accidental operation of the machine: On or about May 8, 2019, in the assembly area, employees were exposed to unintentional activation of the Martin spot welding machine. The spot welder foot pedal protective cover was by passed with rubber bands holding it up away from the activation pedal.
