29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: On or about August 28, 2018, employees are exposed to caught-in hazards due to sprocket and chains missing guards in two instances; a) At the accident site, sprocket and chain gears near and over the fan blade to the rotary cooling machine were missing guards. b) Outside in the production area, sprocket and chains to the chile cleaning conveyor machine was missing a guard.
