29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) Line 3: Production department - On or about June 27, 2018 and at times prior to, employees used an extension cord to energize equipment including but not limited to fans and production machines. The cord hangs from the machine structure and strain relief was not provided. The outer insulation was separated from the attachment plug, exposing the insulated conductors inside. b)Line 2: Enercan machine - On or about June 27, 2018 and at times prior to, employees plugged in the 480 V cable that energized the machine. The cord was hanging from the machine structure and strain relief was not provided where the attachment plug was connected to the cord exposing the insulated conductors inside were exposed.
