29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a certification that contains the date, the location of the space, and the signature of the person making the determination. The certification was not made available to each employee entering the space or to that employee's authorized representative: Boiler 2: On, or about, September 17, 2014, a cleaning crew that worked for a subcontractor was allowed to enter the lower grate section of Boiler 2 for cleaning operations without an Attendant present before the space was reclassified as a Non-Permit Confined Space on Permits No. 14-145 and No. 14-146.
