29 CFR 1915.152(a): The employer did not provide or ensure each affected employee used the appropriate personal protective equipment (PPE), including life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE: a. On or about January 21, 2018, on the pier across from the fabrication shop, employees working along the edge of the pier were exposed to a drowning hazard, in that; personal flotation devices were not being worn. b. On or about January 23, 2018, on the pier across from the electrical shop, employees were allowed to access the vessel, Coast Guard Vessel 45715, by skipping the tires. The employees were exposed to a drowning hazard, in that; they were not wearing personal flotation devices. c. On or about January 24, 2018, on the pier across from the machine shop, an employee working along the edge of the pier was exposed to a drowning hazard, in that; a personal flotation devices was not being worn.
