29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) (DESCRIBE HAZARD(S) WHERE NECESSARY) On 10/27/15, in the maintenance shop of the facility, employees used a Cincinnati bench grinder (serial #230659) that was equipped with work rests set up to an inch away from the wheel.
