29 CFR 1910.119(m)(1): The employer did not investigate each incident which resu lted in, or could reasonably have resulted in, a catastrophic release of a highl y hazardous chemical in the workplace: The employer does not ensure that investigations are conducted on eac h incident that could result in a catastrophic release of highly hazardous chemi cals. a) This violation was documented on or about November 4, 2010, when the em ployer failed to conduct an incident investigation on a leak of a 4 inch block v alve located at the ammonia storage tank 69-V-15 in 2009. This exposed the emplo yees to the hazards of fire and explosion Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of the citation, the employer must submit documenta tion showing that it is in compliance with the standard including describing the steps it is taking to ensure that facility-wide incident investigations are bei ng conducted and the action items that result resolved in a timely manner.
