29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective. a)Engine room: On or about October 4, 2010 and at times prior thereto, a 4.5 Kilovolt panel was not properly grounded. The grounding cable was attached to an out of service water line that had been disconnected at both ends. Note: Abatement certification is required for this item.
