29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process: (a)There was no MOC (management of change) conducted following a 12/22/10 hydrofluoric acid leak incident at the tank U-613 relief header rupture disk, where revised bolt torque procedures were implemented to ensure compatibility with rupture disks, their holders, and gasket materials. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
