29 CFR 1910.119(l)(2)(ii): The written procedures to manage changes did not assure that the impact of the change on safety and health was addressed prior to any change: The employer does not ensure the written procedures to manage changes that impact safety and health was addressed prior to any change. This violation most recently occurred at the Pasadena Refining Systems Inc. facility located in Pasadena, Texas on or about June 30, 2010, and times there to, in the Alky 1&2 and FCC units, where the impact of not monitoring the performance of leak clams and the impact they will have on safety and health was not looked at, exposing employees to possible fire and hazardous chemicals.
