Text For Citation: 01 Item/Group: 015 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and/or crushing hazards: (a) Facility, E. Hillis Street, Youngwood, PA: On or about April 23, 2007, the following deficiencies were noted on yard equipment: (1) CAT 1T28F front end loader: the back up beeper was inoperable, the lights didn't work and there was no load limit posted in the machine. (2) Cement Truck #5197: the battery compartment was missing its cover, the windshield was cracked, no fire extinguisher in cab and the pressure gauge was broken. (3) Cement truck #5198: the battery compartment was missing its cover, the back up-alarm was inoperable, no working headlights and the driver's side door was missing. Among other methods, one feasible and acceptable means of abatement is to repair/replace the deficiencies and develop a preventive maintenance procedure for all yard equipment.
