Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: (a)AFCO Steel LLC at 31455 County Road 39 1/2, Greeley, CO 80631: On or before 03/16/05, the employer did not conduct annual or periodic inspections of the 17 overhead wall jib cranes. As a result, there was no assurance that these cranes, which were used to lift steel and other materials, were in good working order. For example, one jib crane, located near the steel staging area, included a deformed hook, another hook was missing its safety latch, and this crane did not have its rated load capacity stated on it. Moreover, there were no records of inspection. Condition exposed employees to struck by hazards. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Sample Abatement-Certification Letter"). Among other methods, one feasible and acceptable abatement method included conducting periodic inspections, and keeping records of such inspections, in accordance with American National Standards Institute (ANSI) B30.11, "Monorails and Underhung Cranes," and ANSI B30.16, "Overhead Hoists": ANSI B30.11 - 1980 Section 11-2.1.3 provides that the hoist inspection shall be conducted in accordance with ANSI B30.16 - 1973. In addition, ANSI B30.11 provides that a complete inspection of normal service equipment shall be done annually, and specifically lists inspection items to address. Moreover, ANSI B30.11 Section 11-2.1.4 provides that equipment which is used for standby service shall also be inspected annually. Section 11-2.1.5 provides that dated evidence of the inspection of critical items shall be maintained.
