29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) At 4895 Old Louisville Road, Garden City, GA 31408 in Warehouses 1 and 2, and the Lot: On or about December 16, 2022, and times prior to, the employer exposed employees to struck-by hazards, in that forklift operator evaluations were not conducted at least once every three years for employees who operated forklifts on a daily basis to complete loading and unloading operations.
