29 CFR 1910.147(f)(4): Specific procedures were not utilized during shift or personnel changes to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout device protection between off-going and oncoming employees, to minimize exposure to hazards from the unexpected energization or start-up of the machine or equipment, or the release of stored energy: Central Paper Stock, Inc. located at 6665 Jonas Place in Berkeley, MO 63134: The employer is failing to protect employees from caught-in and crushing hazards while performing maintenance, clearing jams or cleaning machines. This condition was most recently observed on or about 11/17/2021, when the employer failed to implement and utilize specific procedures during shift or personnel changes to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout device protection between off-going and oncoming employees, to minimize exposure to hazards from the unexpected energization or start-up of the machine or equipment, or the release of stored energy. Employees failed to implement specific procedures during shift changes to ensure energy control procedures were complied while locking out machines to include, but not limited to, the following : a) The plastic "Badger" baler (Model: BADGER L-100S-2-10/7; Serial No: 8658) on the plastic side of the plant.
