29 CFR 1910.119(l)(3): Employees involved in operating a process and maintenance and contract employees whose job tasks will be affected by a change in the proc ess were not informed of, and trained in, the change prior to start-up of the pr ocess or affected part of the process: The employer does not ensure that employe es whose job tasks are affected by a change in operating process are informed of and trained in the change prior to start-up. a) This violation was documented o n or about November 18, 2010 at Lion Copolymer - Baton Rouge, when the Plant Man ager had a certified programmer make modifications to the operating logic of the SAP computerized maintenance management system (CMMS) to include a "new" priority code for Safety Work Orders. This increased the number of priorit y code choices for corrective maintenance notifications from four to five. The a dditional priority code was added around October 2008. The employees that utiliz e the CMMS do not know about the safety code and do not know how to properly use the SAP system to get necessary maintenance work done. b) This violation was do cumented on or about November 18, 2010, when the employer modified the normal op erating procedures in the Utility Pump House to include operating the ammonia re cycle loop in a bypass condition that was manually controlled by the operator wh en the original design allowed an automated "regulator" to modulate this bypass. c) This violation was documented on or about November 18, 2010, at Lion Copolym er, LLC - Baton Rouge when the employer installed a valve clamp on the valve loc ated at the ammonia knock-out system. d) This violation was documented on or about November 18, 2010 , when the employer required that come-alongs be utilized to open and close manu al valves in the Butadiene Recovery Area. Failure to maintain a properly conduct ed management of change could potentially lead to the hazards of fire and explos ion. Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of this citation, the employer must submit documentation showing that it is in complianc e with the standard, including describing the steps it is taking to correct the management of change to reflect the change in the CMMS and the change in the UPH operating procedures.
