29 CFR 1910.305(g)(2)(iii):Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about August 5th, 2024, employees engaged in commercial bakery production activities located at 4657 G Street, Omaha, Nebraska 68117, were exposed to fire and electrical hazards in that flexible cords in use were not provided with strain relief necessary in preventing damage that occurs from the weight of the cord pulling on the interior terminals and wiring. Instances of failure include but are not limited to: a) 2 damaged flexible cords inserted into Power Flex 40 - White Packaging Production Line. b) 2 damaged suspended flexible cords - White Packaging Production Line. c) 1 damaged flexible cord inserted into Power Flex 4 - White Packaging Production Line. d) 1 damaged flexible cord inserted into Power Flex 40 - Blue Line next to the Blue Oven. e) 2 damaged flexible cords inserted into Power Flex 40 and Icing Control junction Box - Line #2. f) 1 damaged flexible cord inserted into Baldor motor - Oven #2 Feed Conveyor. g) 1 damaged flexible cord inserted into motor - Pan return #1 conveyor. h) 1 damaged suspended flexible cord - Red Moline Line. i) 1 damaged flexible cord inserted into Baldor motor - Red Moline Line. j) 1 damaged flexible cord inserted into junction box under conveyor next to the icing tank - Panning Line. k) 2 damaged flexible cords inserted into Power Flex 40 - Mounted on side of conveyor next to Refrigeration Spiral #2 door. l) 1 damaged flexible cord inserted into white motor - Mounted on side of conveyor next to Power Flex 40 - Next Refrigeration Spiral #2 door.
