Text For Citation: 01 Item/Group: 014A Hazard: EXPLOSION
Section 6(a) of the Puerto Rico Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees in that employees were exposed to: Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico: El patrono no proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten causando o que puedan causar muerte o dano fisico a los empleados, por cuanto empleados estaban expuestos a: API 653 - April 2009 Cover steel storage tanks built to API 650 and its predecessor API 12C. It provides minimum requirements for maintaining the integrity of such tanks after they have been placed in service and addresses inspection, repair, alteration, relocation, and reconstruction. a)At Caribbean Petroleum Refining in Bayamon (CAPECO), employees that worked performing routine task such as tank operator,waste treatment operator,loading rack operator, among others were or could be exposed to flammable and combustible release, fire and or explosion during the performance of there duties. At the tank farm area the employer did not inspected all atmospheric tanks in which they stored gasoline, jet fuel, fuel oil, and diesel in above ground tanks, ranging from 500 to 500,000 barrels to ensure the integrity of such tanks after they have been placed in service. See Binder 3 tab 2b OSHA PRODUCTION No.2 11/20/09 Monthly inspection Checklist for Bulk Storage Tanks page 356, 359, and 362. Among other methods, one feasible and acceptable abatement method to correct this hazard is: 1.Follow API 653 - 6.1 Inspection - General. Periodic in-service inspection of tanks shall be performed as defined herein. The purpose of this inspection is to assure continued tank integrity. Inspections, other than those defined in 6.3 shall be directed by an authorized inspector. API 653 - Abril 2009 Abarca tanques de almacenamiento de acero construidos de acuerdo al API 650 y su antecesor API 12C. Este provee requisitos minimos para el mantenimiento de la integridad de dichos tanques despues de haber sido puestos en servicio y trata inspecciones, reparaciones, alteraciones, relocalizaciones y reconstrucciones. a)En "Caribbean Petroleum Refining" en Bayamon (CAPECO), empleados que trabajaban realizando tareas rutinarias tales como operador de tanque, operador de tratamiento de aguas, operador de area de carga, entre otras fueron opodrian haber estado expuestos a liberacion de productos quimicos inflamables, combustible, fuego y/o explosion durante el desempeno de sus deberes. En la granja de tanque el patrono no inspecciono todos los tanques atmosfericos, donde se almacenaba en tanques sobre el terreno gasolina,diesel,aceite combustible, combustible para aviones con capacidades que fluctuaban de 500 a 500,000 barriles para garantizar la integridad de estos tanques luego de haber sido puesto en servicio. Un metodo seguro entre otros metodos aceptables para corregir este riesgo es: 1.Seguir el API 653 -6.1 General. Inspeccion periodicas de tanques en servicio deben ser llevadas a cabo como definida en este. El proposito de esta inspeccion es para asegurar la integridad continua del tanque. Inspecciones, fuera de aquellas definidas en el 6.3 deben ser dirigidas por un inspector autorizado.s,
