29 CFR 1910.134(c)(1): The employer did not ensure that a written respiratory protection program with worksite-specific procedures was established and implemented to include the provisions of (i)-(ix), as applicable: a) Throughout facility - The written respiratory protection program did not contain specific details including, but not limited to, procedures for respiratory selection, emergency use of the SCBA, procedures for repairing when required, procedures for inspections, procedures to ensure adequate air quality, quantity and flow of breathing air for SCBAs, training topics to address the hazards which they be exposed to under routine and emergency situations and the frequency of fit testing, on or about 2/8/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
