29 CFR 1915.505(e)(3)(i): The employer did not provide a self-contained breathing apparatus (SCBA) to all fire response employees involved in an emergency situation in an atmosphere that was IDLH, potentially IDLH, or unknown. On or about March 15, 2024, onboard the Cuyahoga vessel moored at the Port of Ashtabula, Kinder Morgan Pinney Dock; Minnesota slip, A/B dock side, the employer did not provide employees engaged in firefighting activities in cargo hold #2 with SCBAs exposing employees to asphyxiation hazards.
