29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace: a) At 4895 Old Louisville Road, Garden City, GA 31408 in Warehouses 1 and 2, and the Lot: On or about December 16, 2022, and times prior to, the employer exposed employees to struck-by and caught in hazards, in that powered industrial truck training containing formal instruction and an operator evaluation was not provided to employees that performed forklift operations including loading and unloading of vehicles from trailers and into containers.
