Dock on Frame Scaffold: On or about 12/19/2019, employees were exposed to fall hazards while working from a scaffold platform using a personal fall arrest system that was not capable of supporting a 5000 lb. anchorage point or a complete personal fall arrest system, which maintains a safety factor of at least two.
