29 CFR 1910.217(b)(4)(ii): Mechanical power press(es) using full revolution clutches were not provided with a pad with a nonslip contact area firmly attached to the pedal: On or about March 17, 2015, in the EPTD Press Area of Building #1, the Bliss Press #112 (Model MI/DAC 0016/Serial 0123) installed with a full revolution clutch did not have a nonslip contact area firmly attached to the pedal. The foot pedal had raised grip bumps which became slippery from wear and tear and exposed employees to struck-by and crushing injuries.
