(c)Date of Violation:04/23/13 Location:Throughout the Storage Yard and at the Storage Sheds, Work Sheds, and Work Shops Equipment:Datsun 7000 Forklift, Model-YF03A33TV, Serial #-YF03-000717 Condition:The forklift was operated with the seatbelt wrapped up under and behind the seat so it could not be used.
