(b) West Electrical Room at USDA D4-1 and D4-2 Panel - On or about July 31, 2023, the opening around two 480-volt electrical disconnect levers was not covered exposing employees to an electrical hazard. (c) West Electrical Room at C1-2 Stack Off Conveyor Panel - On or about July 31, 2023, an open hole in a 480-volt electrical cabinet was not covered exposing employees to an electrical hazard.
