29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l). On or about April 20, 2022, and at times prior thereto and thereafter, at the jobsite located at 1501 Scripture Street, Denton, TX 76201, employees who operated the rough terrain style forklift(s) at the jobsite were trained, evaluated and certified prior to operation exposing employees to roll-over and struck by hazards.
