29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown was not utilized to avoid any additional or increased hazard(s) to employees: Central Paper Stock, Inc. located at 6665 Jonas Place in Berkeley, MO 63134: The employer is failing to protect employees from caught-in and crushing hazards while performing maintenance, clearing jams or cleaning of machines. This condition was most recently observed on or about 10/26/2021, when an employee was fatally injured while attempting to clear a jam on a large "Harris Selco" paper baler (Model: HLO-8221AR-150; Serial No: 39770079). The employer failed to ensure the machine or equipment was turned off or shut down using a procedure for the machine.
