Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA, 15824, production area, on or about January 8, 2016 and times prior thereto: Employees utilize the Federal No. 7 powder metal press (Serial Number: 7-727, M1) for purposes including, but not limited to, resizing rings that were out of round. The fly wheel was 6-feet above the floor surface and was not guarded on the left side of the flywheel.
