29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about September 14, 2015, employees were exposed to amputation hazards when the grinder head exit end was not guarded. An employee's finger was amputated by the blade when he reached into the end of the machine. No abatement certification or documentation is required for this item.
