29 CFR 1910.333(b)(2)(i): The employer did not maintain a written copy of the procedures outlined in paragraph (b)(2) and did not make it available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives. a. A&D Wood Products, Inc. located in Elida, Ohio: On or about September 5, 2014, the employer did not maintain electrical safety related work procedures which describe how electrical equipment is going to be deenergized, locked out, verified of deenergization, and reenergized. 29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards were not provided with, and did not use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed. a. A&D Wood Products, Inc. located in Elida, Ohio: On or about October 4, 2014, the employer did not ensure an employee who was exposed to electrical arc flash hazards when changing out fuses in the 240 volt electrical cabinet for the saw was wearing arc rated clothing. b. A&D Wood Products, Inc. located in Elida, Ohio: On or about October 4, 2014, the employer did not ensure an employee who was exposed to electrical arc flash hazards when changing out fuses in the 240 volt electrical cabinet for the swing saw was wearing heavy leather gloves. c. A&D Wood Products, Inc. located in Elida, Ohio: On or about October 4, 2014, the employer did not ensure an employee who was exposed to electrical arc flash hazards when changing out fuses in the 240 volt electrical cabinet for the swing saw was wearing a hard hat. d. A&D Wood Products, Inc. located in Elida, Ohio: On or about October 4, 2014, the employer did not ensure an employee who was exposed to electrical arc flash hazards when changing out fuses in the 240 volt electrical cabinet for the swing saw was wearing and arc rated face shield.
