The specific safety-related work practices were not consistent with the nature and extent of the associated electrical hazards. a. A&D Wood Products, Inc. located in Elida, Ohio: On or about October 4, 2014, the employer did not ensure employees used electrical safety related work practices when changing fuses inside of the 240 volt electrical cabinet for the swing saw.
