29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) On or about May 12, 2016, in Building 155, Layout Department, employees were exposed to struck by, caught-in and/or amputation hazards when walking in the rear of the Chicago Shear machine, due to no barrier guarding in place. b) On or about May 12, 2016. in Building 155, Column B:8, employees were exposed to struck by, caught-in and/or amputation hazards when working in front of the Vane Machine, where the hydraulic press was located with no guarding in place. c) On or about May 12, 2016, in Building 155, in the Nozzle Department, employees were exposed to unguarded parts of the marvel horizontal saw exposing the employees rotating parts and amputation hazards. d) On or about May 31, 2016, in Building 115, Filter Tex Lines; employees exposed to rotating parts, caught-in, and amputation hazards on the Rando Feeders for all Lines, Line 301, 302, and 303 due to inadequate guarding on rotating parts. e) On or about May 12, 2016, in Building 155, employees were exposed to struck by, caught-in and/or amputation hazards when operating the Dayton 20-inch drill not being guarded. f) On or about May 11, 2016, in Building 135, employees were exposed to struck by, caught-in and/or amputation hazards when operating the Carlton drill due to no guarding in place. g) On or about May 11, 2016, in Building 135, employees were exposed to being struck-by, caught-in and/or amputation hazards when operating the Summit 20-inch drill due to no guarding in place. h) On or about May 31, 2016, in Building 115, the Core department, employees were exposed to being struck-by, caught-in and/or amputation hazards when operating the Rockford Belt Sander due to inadequate guarding on the unused portion of the belt. i) On or about May 31, 2016, in Building 115, in the Core Department, employees were exposed to being struck-by, caught-in and/or amputation hazards when operating the MSC Metal Cutting Band Saw due to no guarding in place. j) On or about May 31, 2016, in Buildings 115, employees exposed to being struck-by the Lantech Stretch Wrap Machine, due to no barrier guarding in place. k) On or about June 1, 2016, in Building 197, employees exposed to being struck-by the ITW Mima Stretch Wrap Machine, due to no barrier guarding in place. l) On or about June 1, 2016, in the MDPTC Building, PEM Line machine, employees were exposed to being caught-in and/or amputation hazards from the Transport Belt Roller having two openings not guarded. m) On or about May 12, 2016, in Building 130, employees operating the Sub-arc machine were exposed to rotating parts and caught-in hazards from the side panel having no guard in place.
