29 CFR 1910.265(c)(3)(iii): Floor(s) were not kept in good repair: (a) Debarker: On or about October 21, 2015, and times prior, the floor/walkway on the side of the debarker was sloped and uneven resulting in the floor leaning heavily inwards towards the debarker control cab. (b) Sawmill: On or about October 21, 2015, and times prior, the floor on the outer edge of the sawmill was broken and not secured, along with the mill floor throughout containing loose boards, holes/openings, sagging areas, and weak spots. (c) Planer Area: On or about October 21, 2015, and times prior, the floor on the outer edge of the planer area was broken and not secured, along with the floor throughout containing holes, opening, and weak spots.
